Gail Pe’ay, MHA, PCHA, ALRA, Executive Director

Gail Pe’ay worked in the senior living industry in a wide variety of roles from 1996 until landing her current position as Executive Director at Heartis Yardley. She is pleased to be working with an incredibly knowledgeable and passionate team with decades of experience. Her goal is to add joy and meaning to our residents’ lives while creating an environment of trust, respect and safety.

Gail is a wife, mother of four and grandmother. She recently received a master’s degree in health care administration, graduating at the top of her class with honors. Outside of work, Gail enjoys shopping, traveling and spending time with friends and family.

Yardley Sean Feijo, Business Office Director

Sean Feijo, Business Office Director

Sean Feijo has worked in senior living for over 12 years and brings a wide range of experience to his position as Business Office Director. He has worked within a variety of environments, from a skilled nursing setting to independent living and assisted living communities. He has also gained experience in a variety of roles. Starting as a dishwasher, he worked his way up to Food Service Director, and he also assisted in the Activities department. Sean also worked as certified nursing assistant and receptionist, and it was this experience that led him to his home in the Business Office, where he worked for six years. After serving as the Executive Director for Heartis Yardley, Sean is excited to return to his previous role of Business Office Director.

Sean combines this variety of knowledge with his passion for helping others to positively impact everyone he encounters at Heartis Yardley. In his free time, Sean enjoys cooking, football, playing the drums, traveling, and spending time with friends and family.

Chiffoni Puckett, LPN, CDP, Resident Care Director

Certified Dementia Practitioner

Chiffoni Puckett brings 15 years of health care experience to her role as the Resident Care director for the Heartis Yardley team. She previously worked in skilled nursing, behavioral health and memory care. Chiffoni began her career as a home health aide before becoming an EMT and successfully growing to hold director roles. She is passionate and empathic about her career with the senior population.
Chiffoni is a proud mother of three sons and is a very active soccer mom. She resides locally in the Yardley area. Chiffoni is an avid movie-goer and enjoys décor and art-related hobbies.

Goal: To keep learning and evolving.

Monica Howard, LPN, CDP, Generations Program Director

Certified Dementia Practitioner

Monica Howard comes to Heartis Yardley with 10 years of senior living experience with an emphasis in memory care.
Monica began her health care career as a caregiver and progressed to medication technician and then wellness nurse, spending the past five years working as the assistant director of Nursing in assisted living/memory care communities. Monica places a special emphasis on memory care and specializes in facilitating dementia-based support groups and trainings. She loves how her job allows her to enrich the lives of residents through meaningful and fulfilling programming and activities.
Originally from Philadelphia, Monica enjoys reading, writing and music. In her free time, she loves traveling and playing games with her 10-year-old son.  

Daily Goal: Make a difference in someone’s life.

Yardley Patrice Y. Martin, Director of Sales & Marketing

Patrice Martin, Director of Sales & Marketing

Patrice draws upon her extensive background in sales and marketing when connecting with future residents, families and community partners. Having specialized in senior living for the past six years, she is guided by her passion to serve others. Patrice learns the life stories of residents so she can contribute to supporting their needs and interests. Her experience in residential, independent and assisted living communities enhances her dedication to finding the perfect home to complement and enrich the lifestyle of residents. A native of Bucks County, Patrice returned home after several years of traveling to be near her family, raise her two children and serve the community she loves. She starts her day with meditation, prayer and coffee and also enjoys gardening, riding her bike and traveling.

Yardley Matthew Nguyen, Director of Sales & Marketing

Matthew Nguyen, Director of Sales & Marketing

With 10 years of health care and senior living experience, Matthew finds it truly rewarding to witness the organic relationships being built with each resident as they, in turn, influence the growth of a tight-knit community. As a result of his resident-centered approach, Matthew thoughtfully connects with prospective residents and their families, values their needs, and offers solutions responsive to those needs. These experiences, coupled with his empathetic nature, allow him to gain a valuable understanding of the challenges residents experience when planning their next step. Matthew’s passion for those he serves helps him paint a tailored and vivid picture of the endless possibilities when moving to Heartis Yardley. Outside of serving the community, Matthew enjoys winding down by spending time with family, traveling and playing golf.

Diane Evans, Vibrant Life® Director

Diane Evans, the Vibrant Life® director at Heartis Yardley, brings over 13 years of experience in the senior living and health care industry to the position. Diane is strongly committed to service, and she enjoys providing residents with vibrant and engaging activities that enhance every aspect of their lives. She loves that her work allows her to put smiles on all of the residents’ faces.

Diane lives in South Philadelphia, and when she’s not working, she enjoys spending time with family and friends.

Daily Goal: “Always try to leave people better than you found them. Hug the hurt. Kiss the broken. Befriend the lost. Love the lonely.”

Denise Sanchez RCD headshot

Denise Sanchez, CDP, Resident Care Coordinator

Certified Dementia Practitioner

As the Resident Care coordinator at Heartis Yardley, Denise Sanchez oversees nursing, recreation and memory care. She brings nearly 20 years of experience in health care management, including social services, to the position. She is extremely passionate about memory care and is a Certified Dementia Practitioner, Master Dementia Trainer and Certified Cognitive Stimulation Instructor. Denise strives to be a positive leader and train individuals to deliver care with compassion.

Denise lives in Abington, Pa., with her four sons. Together they enjoy exercising, playing board games, watching movies and spending time outdoors.

Goal: Do my best to help others succeed.

Yardley Matthew Cummins, CDM, Culinary Services Director

Matthew Cummins, CDM, Culinary Services Director

With over 10 years in the culinary field, Matthew brings the expertise and service of a restaurant to the Heartis Yardley community. He is a graduate of the University of Delaware HRIM program and an accredited Certified Dietary Manager. With his experiences as a restaurant manager, head chef and co-founder of the local Johnson Hall Coffee House, Matthew utilizes his passion for fresh food and caring service to deliver delicious meals daily. Matthew has a special passion for brunch and specializes in crepes with fresh whipped cream, scratch-made compotes and of course, a la mode. When not serving the residents, he spends time hiking, gardening and enjoying the outdoors with his family.

Seneca Smith, BSD

Seneca D. Smith, Building Services Director

Seneca Smith brings over 15 years of experience in building services and maintenance to Heartis Yardley. He has been privileged to previously serve in multiple leadership positions and work within a variety of assisted living companies and skilled nursing facilities. Seneca is passionate about building warm and trusting relationships with residents and families, as well as colleagues. Seneca’s background in the United States Marine Corps helped inform the strong leadership he brings to his position with Heartis Yardley.

In his free time, Seneca can be found coaching softball, relaxing at the beach or watching his beloved Phillies. Seneca has two wonderful children, with whom he spends most of his time outside of work.
